The Context: Your team is responsible for a critical weekly data report for a major client. This report must be 100% accurate.
The Incident: It is Friday at 4:30 PM. A senior manager calls you, furious. The report sent this morning contained a major calculation error that misled the client’s executive team.
The Discovery: You investigate and find the error was made by Sarah, one of your most reliable team members. Sarah has been under a lot of personal stress lately and stayed late last night to finish the report alone because the rest of the team left at 5:00 PM.
What will be your response?
Option A (Compliance Mindset): Call Sarah into your office immediately, show her the error, and issue a formal warning. Reiterate that “100% accuracy is the standard” and this cannot happen again.
Option B (Quality Mindset): Check in on Sarah first. Acknowledge the error as a team failure, and ask, “What happened in our process that allowed this error to get through?”.
